The patient-centered medical home is a health care delivery model based on the relationship between a patient and their personal primary care physician. The physician leads a health care delivery team dedicated to providing comprehensive and continuous care.

Across the entire health care system, patients receive the right care at the right time, lowering their health care costs and improving their overall health. Employers pay less for health care as their employees spend fewer hours out of the office. Physicians are paid appropriately for providing coordinated, on-going care.

PCPCC releases report on medical home pilot studies

The Patient Centered Primary Care Collaborative released a report detailing work around the country to study the effectiveness of patient-centered medical homes. "Proof in Practice: A compilation of patient centered medical home pilot and demonstration projects" reports the progress of pilots and initiatives in 18 states, providing descriptions and details of how they are structured and what they are achieving. Cigna launches PCMH pilot project in Dallas-Fort Worth area

Cigna HealthCare announced on Aug. 28 that it will partner with Medical Clinic of North Texas to launch a pilot program aimed at improving quality and reducing health care costs of patients with chronic conditions. To support the patient-centered medical home program, Cigna hired a care coordinator to help the clinic’s 82 doctors find cost-effective ways to treat Cigna’s 10,000 patients.
